Friends & Family Signup - Step 1

We are happy to welcome you as a client and take your health challenges very seriously. You are not just another number to us. Our goal is to partner with you to meet your specific health needs through education, lifestyle and dietary changes along with a customized plan of nutritional supplementation. About Your Body, Inc. offers a broad range of the most technologically advanced services available today.

Sometimes due to client proximity to our office or financial constraints, health can get put on hold! This is the last thing we want, and so we have come up with this solution. We call it the Friends & Family Program.

This is how it works:

For just $59.95 per person, each patient can fill out our Health Consultant Status Form. When the form has been filled out, our practitioner, Daniel Phend, will review your form information and then at a time convenient for the patient, Daniel will call you and give you a 25 minute consultation reviewing your form results and recommending a course of action that will promote your health and well-being.

First, click add to cart, and pay for the number of patients participating in the friends and family program. Be sure to update the number based on how many patients will be taking part in our program.

*Note: This program is NOT for the blood testing specials!!  This is a separate program for  those who want a health consultation with Daniel Phend, over the phone rather than in clinic, and is designed for anyone who wants to take advantage of this opportunity.

Step 2

Once you have paid, please Fill in this form for each patient who is filling out the health consultant status form. Once this form is completed, submit the form, and then fill out the health consultant status form for each patient. Questions can be directed to assistant@aboutyourbody.net.
